A hazard sign is a signboard that warns you a risk is present at that spot — a yellow triangle with a black pictogram and a black border, under BS EN ISO 7010 and the UK's Safety Signs and Signals Regulations. In American workplaces the same job is done by a rectangular DANGER or CAUTION sign under 29 CFR 1910.145. Those two systems are not the same, and the red-bordered diamond on a drum of solvent is a third thing again.

That confusion matters more than it sounds. Across fifteen years and fourteen countries I have walked semiconductor fabs, bottling halls, mine camps and city-centre construction pads. The signage findings I write up are almost never "there was no sign." They are: the sign was the wrong category, the sign outlived the hazard, or nobody had ever been told what it meant. What a hazard sign is, and what it is not

A hazard sign communicates a specific risk through a combination of geometric shape, safety colour and a pictogram, so that it works before anyone reads a word of text. That triple redundancy is deliberate: shape and colour are recognised at distance and at speed, the pictogram carries the specific meaning, and any text is a supplement rather than the message. It is why a shift worker with no English can still read a wall.

The legal definitions are narrower than everyday usage. In Great Britain, a warning sign means a sign giving warning of a risk to health or safety, and a signboard means a sign that provides information or instruction by a combination of geometric shape, colour and a symbol, rendered visible by lighting of sufficient intensity. In the US, 1910.145 defines a sign as a surface prepared for the warning of, or safety instruction of, workers or the public — and explicitly excludes safety posters, bulletin boards and news releases from that definition.

Signs, tags, labels and markings are four different objects

Practitioners use these interchangeably in conversation and then find they are governed by four different instruments. A tag is temporary and travels with an abnormal condition; a signboard is permanent and belongs to the place; a chemical label belongs to the container; a floor or edge marking belongs to the geometry of the space.

The table below separates the four, because getting this wrong is the most common reason a signage audit finding gets argued about:

Three practical consequences follow from that split, and each one has cost someone a finding on a site I have audited:

  • A tag is not a sign. Under 1910.145(f)(3), tags are used until the hazard is eliminated or the operation is complete — a tag still hanging six months later is evidence the abnormal condition became normal.
  • A drum label does not signboard a room. Storage areas for significant quantities of hazardous substances need a warning signboard at the door or nearby, unless the labels on individual containers are clearly visible from outside.
  • A marking is not a barrier. Stripes tell you where an edge is; they do not stop you reaching it, and they carry no weight in a fall-prevention argument.

The five types of hazard sign, with examples

Ask most people to name the types of hazard sign and you get the ISO/European set of five, because that taxonomy has spread worldwide through ISO 7010 even where it has no legal force. Each type is defined by its "intrinsic features" — the shape, the colour, and the minimum proportion of the sign that colour must occupy. Get the proportion wrong and the sign is technically non-compliant even when the pictogram is perfect.

Prohibition signs — round, red, diagonal bar

These prohibit behaviour likely to cause a risk to health or safety. A black pictogram sits on a white background inside a red ring with a red diagonal line running across it. Examples: no smoking, no access for unauthorised persons, no access for pedestrians, not drinkable, do not extinguish with water, do not touch.

Warning signs — triangular, yellow, black border

The fastest way to pick the right category is to ask what verb the sign carries:

  • Forbids an action → prohibition, red ring and bar
  • Warns of a hazard without forbidding or requiring anything → warning, yellow triangle
  • Requires an action → mandatory, blue circle
  • Shows a safe condition or a way out → emergency escape, green rectangle

Warning signs: what the yellow triangles cover

The yellow triangle set is the narrowest and most misused category on most sites. Two rules govern it and both are routinely broken. First, the pictogram must match the actual hazard; second, the general danger sign — the yellow triangle with a black exclamation mark — must not be used to warn about hazardous substances or mixtures, except where it flags a store holding several different ones.

I see the general danger triangle used as a shrug. It goes up when nobody is confident which specific sign applies, and the effect on a reader is close to zero: it tells them something is wrong here without telling them what to do differently. If no registered symbol fits, ISO 3864-1's answer is to use the general symbol with supplementary text, not on its own.

The sign that was deleted and is still on walls

One warning sign no longer exists and still turns up on doors and cabinets: the black St Andrew's cross for "harmful or irritant material." It was removed from the Regulations by the CLP (Amendment) Regulations 2015, and HSE's guidance says plainly that where employers used it previously they should now use the most relevant remaining warning sign. It is still sold. I still find it, usually on a chemical store door that has not been reviewed since the store was built.

Three checks settle whether a yellow triangle on your wall is doing its job:

  • Is the pictogram specific? A general danger triangle standing in for a specific hazard is a placeholder, not a warning.
  • Is it a sign that still exists? The harmful or irritant cross was deleted in 2015 and should have been swapped out.
  • Does it carry supplementary text where the symbol is unfamiliar? Text reinforces meaning best when its background colour matches the sign it supports.

Hazard pictograms: the diamonds that are not signboards

The red-bordered diamond on a drum belongs to a different legal system entirely. Those are GHS hazard pictograms, applied to chemical containers and pipes under the CLP Regulation in Great Britain and the EU, and under the Hazard Communication Standard in the US. They are label elements — square set at a point, black symbol on a white background, red frame wide enough to be clearly visible, per Appendix C to 1910.1200. A red frame with no symbol inside it is not a pictogram at all and is not permitted on a label.

There is a numerical trap here that catches even experienced practitioners. CLP lists nine hazard pictograms. OSHA's HazCom standard designates eight — the environmental pictogram is not required, because environmental harm falls outside OSHA's remit. Anyone writing one global chemical labelling procedure needs that difference on the first page.

Auditing water treatment plants across Ireland, France and Spain, chemical store signage was where paperwork and reality parted company most often. The store door carried a general danger triangle; inside were chlorine cylinders, a caustic bulk tank and an acid IBC, each with the correct diamond on the container, none of it visible from outside. Legally the triangle was defensible for a multi-substance store. Operationally it told an incoming responder nothing about which cylinder was going to matter first.

If you operate in the US, two dates are live right now. OSHA's 2024 HazCom final rule aligned the standard with GHS Revision 7, and in January 2026 the agency pushed its compliance dates back by four months.

The revised employer deadlines to work to are:

  • 20 November 2026 — update workplace labelling, the written hazard communication programme and employee training for substances
  • 19 November 2027 — manufacturers, importers and distributors evaluating mixtures comply with the modified provisions
  • 19 May 2028 — the equivalent employer labelling, programme and training deadline for mixtures

US, UK and ISO: three sign systems compared

The five-type list that dominates search results is the ISO and European taxonomy. American workplaces run a different one, and presenting either as universal is how a multinational procedure ends up wrong in half the countries it covers. Under 1910.145 there are three sign classifications — danger, caution and safety instruction — plus biological hazard signs and the slow-moving vehicle emblem. There is no federal "warning" sign class.

"Warning" does exist in the US, in two places. It is an optional tag signal word for a hazard level between caution and danger. It is also a full category under the voluntary consensus standard ANSI Z535.2. That standard sets DANGER for a hazard that will cause death or serious injury, WARNING for one that could, CAUTION for minor or moderate injury, and NOTICE for messages unrelated to personal injury. Since a 2013 direct final rule, ANSI-style signs are permitted alongside OSHA-style ones for the colour specifications in 1910.145(d).

That permission is not universal. Substance-specific standards keep their own signage wording — asbestos regulated areas under 1910.1001 are the classic example — so an ANSI-format sign does not cure a wording defect there.

Three things I check first on any multi-country signage standard:

  1. Which taxonomy the site is actually built to — mixing a blue mandatory circle into an ANSI-format wall is legal nowhere in particular and confusing everywhere.
  2. Whether the "5 types" list has been copied into a US procedure — if the document says prohibition, warning, mandatory, emergency and fire, it was written for Europe.
  3. Whether substance-specific wording has been overwritten by a house sign template.

Where hazard signs sit in the hierarchy of controls

A hazard sign is the last thing you reach for, not the first. British law is unusually explicit about this: a sign is required only where the risk assessment shows that, having adopted all appropriate collective protection measures and safe systems of work, the employer still cannot avoid or adequately reduce the risk without one. HSE's guidance puts it in one sentence — safety signs are not a substitute for other methods of control. The US rule reaches the same place from the other direction: tags need not be used where signs, guarding or other positive means of protection are in place.

That ordering has a hard operational consequence. If a sign is doing the work a guard should be doing, the sign is a symptom, not a control — and it will fail in exactly the conditions where you needed it most, because compliance with a sign is voluntary in a way that a locked gate is not.

I found a worn shortcut cutting an active haul route on a cement quarry approach in Ireland. There was a marked walkway, correctly striped, twenty metres away. Nobody used it, because it added distance to a walk people made a dozen times a shift. The desire path was visible from space, and the response on offer was another sign.

I closed the shortcut physically, reinstated the marked walkway on the line people were already walking, and briefed shift control. Crossings on that sector dropped on follow-up walks. The transferable lesson is the one I now open every signage review with: where a sign is competing with a shortcut, the sign loses. Move the control, or move the route.

The order to work through before a sign is even a candidate:

  1. Eliminate the hazard or the need to approach it.
  2. Substitute a less hazardous substance, process or layout.
  3. Engineer a guard, an interlock, a barrier, a containment.
  4. Organise the work — permits, exclusion zones, scheduling, competence.
  5. Sign and mark the residual risk, and train people in what the sign means.
  6. Protect the individual with PPE, which the mandatory blue signs then reinforce.

Why hazard signs stop working on real sites

Human factors research treats a warning as a chain rather than an object: it has to be noticed, then understood, then acted on, and it fails completely if any link breaks. A review of safety sign comprehension and compliance sets out the same sequence. Almost every signage finding I write up is a broken link in that chain, and the sign itself usually looks fine in a photograph.

The sign that outlives the hazard

Schedule 1 requires that a signboard be removed when the situation it refers to ceases to exist, and HSE's guidance adds that leaving it up displays misleading information. This is the most-ignored line in the whole instrument. Every stale sign teaches the workforce that signs describe history rather than conditions, and that lesson transfers to the signs that still matter.

The adjacent version of this is a marking that lies. On a cement mill outage at a Belgian site, I traced a LOTO sheet that pointed to the wrong isolation valve on a legacy unit — a labelling failure rather than a signboard failure, but the same mechanism. I stopped the job, corrected the labelling and the map, and required field verification before restart. Wrong-label findings across that unit family were then cleaned in a sweep. A map that lies is worse than no map, because it removes the instinct to check.

The sign that points at a blocked route

At 01:35 in a Dublin bottling hall I traced a CIP hose pinned across a marked exit leaf. The green running-man sign above the door was correct, illuminated and maintained. The route behind it was not available. I stopped the sequence, had the hose rerouted, and added exit checks to the sanitation start card; exit obstruction findings fell on subsequent night cleans.

Emergency signage is the category where the gap between the sign and the world does the most damage, because nobody discovers the gap until the alarm sounds. Two adjacent rules are worth knowing: a UK fire safety sign bearing only the text "Fire Exit" is not acceptable — it needs the pictogram — and a directional arrow is not acceptable on its own.

Sign clutter and habituation

The Regulations state directly that placing too many signs too close together should be avoided, and list poor design, insufficient number, incorrect positioning and poor state of repair as things that degrade a sign's effectiveness. Clutter is the failure that gets designed in by good intentions: every incident adds a sign, nothing ever removes one, and after three years the wall by the door carries fourteen messages that a person walking past reads as texture.

  • Noticing breaks down through clutter, poor positioning, faded material and low light

Choosing, placing and maintaining hazard signs

Selection follows the risk assessment rather than the catalogue. Work out what residual risk remains after the other controls, decide what a person needs to do differently because of it, then pick the category that matches that verb: forbid, warn, require, or direct to safety. If the answer to "what should they do differently?" is nothing, you do not need a sign — you need to look again at the control.

Placement is where good signs die. Signboards go at a suitable height and in a position appropriate to the line of sight. For a general hazard that means the access point to the area; for a specific hazard it means the immediate vicinity of the thing itself. Either way the sign needs lighting adequate for the intended viewing distance. In the US, a tag's signal word must be readable at a minimum distance of five feet — 1.52 m — or further where the hazard warrants, and tags must be fixed as close as is safely possible to the hazard.

The training duty nobody budgets for

Both jurisdictions carry a standalone duty to explain signs. Regulation 5 of the UK Regulations requires suitable and sufficient instruction and training in the meaning of safety signs and the measures to be taken in connection with them. 1910.145 requires that all employees be instructed that danger signs indicate immediate danger and caution signs a possible hazard, and that all employees be informed as to the meaning of the various tags in use.

New, young, agency and non-native-language workers are the population this duty exists for, and the population that induction most often rushes. Mine emergency preparedness work taught me to test it the cheap way: pick the least common sign on the wall and ask three people what it means. If two of them guess, the sign is decoration.

Run these checks on a signage walk, in this order:

  1. Stand where the worker stands. Look for the sign from the approach, at the height and speed people actually travel.
  2. Match the pictogram to the hazard, not to the sign that was in stock.
  3. Walk the route any escape or equipment sign points at, on the shift when it is most likely to be obstructed.
  4. Count the signs in one field of view; anything over about four at an entry point needs a prioritisation decision.
  5. Check legibility under failure — power off, poor light, wet, dusty, at night.
  6. Ask three people what it means, and record what they said rather than what the sign says.
  7. Check the removal trigger — who takes this sign down, and what event tells them to.

So the useful question is not which signs to buy. It is whether the signs already on your walls are still telling the truth, and whether anyone has ever been told what they mean.
